
Once you or your referral source has contacted our office, a therapist will call you to schedule an initial visit in your home to do an initial evaluation.
During this visit, the therapist will evaluate you. A customized treatment plan will be developed based on this evaluation with input from you, your physician, and if appropriate, your family, and caregivers. This team will together develop your plan of care. Your therapist will work with you to set goals for your therapy and to design a home program for you.

We are set up as an outpatient Physical and Occupational Therapy clinic. Medicare Part B covers 80% and if you have a secondary insurance, often referred to as a supplemental policy, it usually covers the remaining 20%. So if you have Medicare Part B and a supplemental policy, services may be covered at 100%. Individual policies vary.
If you qualify as being "homebound" - meaning you go out very infrequently due to physical restraints or cognitive deficits - then we can see you in your home where most insurance plans will provide 100% coverage.
We are able to provide these services by special relationships we have with other homehealth care providers if you also need nursing services.
Just tell the discharge planners (and/or your physician) that you are requesting our services once you leave their facility. They will contact us and arrange for a smooth transition for your continued rehabilitation in your home.
Yes, they can phone in the referral to 301-662-1997 if in Frederick County, Maryland. They can call TOLL FREE if outside Frederick County (1-877-727-3422). They may choose to simply fax us your referral to 301-668-2202.

No, Maryland is one of 46 states that have direct access for physical therapy services and do not require a physician's order to see clients. Some insurance providers however do require a physician's order for reimbursement. If you are paying privately, this of course is not an issue.
