Orthopedic Total Joint Program
Coordinate with attending physicians to provide pre-op in home safety evaluation, give recommendations for home modifications and instruct pre-op home exercise program.
Assist patients who have undergone joint arthroplasty and their caregivers as they transition to home from the hospital or inpatient rehab program.
Establish that the patient is medically stable and that home is the appropriate placement.
Educate client on procedure to be preformed, the impact on quality of life, and expected outcomes.
Provide PT, OT, SN therapies as appropriate and establish realistic goals and time frame in which goals will be achieved.
Prepare patient for transition to outpatient services as appropriate.
