If you are considered "homebound", you can receive
physical therapy and occupational therapy
in your home. No need to go out to a clinic,
we bring the clinic to you… that’s service and
convenience! You can also receive speech
therapy.
What to Expect?
Once you or your referral source has contacted
our office, a therapist will call you to schedule
an initial visit in your home to do an initial
evaluation.
Based on the therapist's evaluation, a customized
treatment plan will be developed, with input
from you, and if appropriate, from your physician,
family, and caregivers. This team will together
develop your plan of care. Your therapist will
work with you to set goals for your therapy
and to design a home program for you.

Q. What insurance does HomeCare Rehab and Nursing,
LLC take?
A. MEDICARE….and some other insurance companies.
Call our office for
specific requests.
Q. Are these services covered 100%?
A. We are set up as an outpatient P.T. and O.T.
clinic. Medicare Part B covers 80% and the 2nd
insurance usually covers the remaining 20%,
so having Medicare and a 2nd insurance may provide
100% coverage. Individual policies vary.
If you qualify as being “homebound” - meaning
you go out very infrequently due to physical
restraints - than we can see you in your home
and most insurance plans will cover this 100%.
What if I need nursing services?
We are able to provide these services by special
relationships we have with other homehealth
care providers if you also need nursing services.
How do I make a referral from the hospital
or nursing home as I plan on going home?
Just tell the discharge planners (and/or your
physician) that you are requesting our services
once you leave their facility. They will contact
us and arrange for a smooth transition for your
continued rehabilitation in your home.
Can my physician make a referral for me
from their office?
Yes, they can phone in the referral to 301-662-1997
if in Frederick County, Maryland. They can call
TOLL FREE if outside Frederick County (1-877-727-3422).
They may choose to simply fax us your referral
to 301-668-2202.
How do I request Dementia Therapy or any
other specialized service?
Just inform us of your request at the time
of your call or referral so we can make sure
the right therapist works with you.
Do I need a physician’s referral to see
a physical therapist in Maryland?
No, Maryland is one of 40 states that have
direct access for physical therapy services
and do not require a physician’s order to see
clients. Some insurance providers however do
require a physician’s order for reimbursement.
If you are paying privately, this of course
is not an issue.
