Dr. John Baker, Director/Owner

John Baker, PT, MA, GCS, NCS, NDT, DScPT received his Masters of Art in College Science Teaching from the University of Maryland in 1986 and his Masters of Science in Physical Therapy from Old Dominion University in 1990. His Masters thesis paper and research related to assessment and treatment of the "Diabetic Foot". He has specialized in home health rehabilitation and has made over 25,000 home Physical Therapy visits in the Frederick region. He has been trained in N.D.T./Bobath in the treatment of Adult Hemiplegia in 1994. In June of 2004, he became a Board Certified Specialist in both Neurologic Physical Therapy and Geriatric Physical Therapy from the American Physical Therapy Association. As of June 2004 he was the only Board Certified Specialist in Geriatrics or Neurology practicing in Frederick County, Maryland. In 2005, we were able to add Speech Therapy for our clients who are homebound.

Dr. Baker received his Doctorate of Science in Physical Therapy from the University of Maryland School of Medicine in December of 2004. He founded HomeCare Rehab and Nursing, LLC in 2002 which provides Physical Therapy and Occupational Therapy services for Medicare and Blue Cross, and other insurance provider clients in their home and assisted living facilities without the restriction of needing to be homebound.

Dr. Baker was elected by fellow colleagues to be the Western District chairman of the Maryland chapter of the American Physical Therapy Association for calendar years 2006 and 2007.

Dr. Baker's appointment by Maryland Governor Martin O'Malley to the Maryland Physical Therapy Board of Examiners has been extended to run from 2008-2012.


Physical Therapists

Gregory Adams, PT, MS, CCS

Gregory Adams, PT, MS, CCS received his Baccalaureate and Masters of Science in Physical Therapy from Thomas Jefferson University in Philadelphia, Pennsylvania. He trained at Rancho Los Amigos in Los Angeles, California. He received his Baccalaureate of Science in Biology from Messiah College in Grantham, Pennsylvania.

Greg has been very involved in cardiopulmonary physical therapy since 1992. He has been certified in Advanced Cardiac Life Support (ACLS) since 1993 and obtained certification as an "Exercise Specialist" (cardiopulmonary rehab) from the American College in Sports Medicine in 1995. Greg received his Cardiovascular and Pulmonary Clinical Specialist through the APTA in 2003.

He has worked in teaching hospitals, including Temple University Hospital, Albert Einstein Hospital (Philadelphia), and still presently at the University of Maryland Medical Center. Greg has taught for physical and occupational therapy programs. Greg teaches critical care conferences nationwide on a regular basis. He is working on blood pressure research of professional publication.

Greg has a wife, son and twin daughters and is an avid lacrosse fan.

Monica M. Armagost, PT

Monica M. Armagost, PT graduated from the University of Maryland at Baltimore, School of Medicine with a Bachelor of Science in Physical Therapy in 1993. She started her career in the acute care setting, seeing patients in the hospital and also seeing out-patients with orthopedic diagnoses and wound care issues. After 2 years Monica decided to move to the sub acute/rehabilitation setting and practiced primarily with patients with recent stroke, heart attack, total joint replacement, Parkinson’s disease, amputation and pneumonia. After 8 years in these settings, Monica has moved into providing home care. Over the years, Monica has taken several courses on physical therapy intervention in rehabilitation. Most recently, she has attended courses in women’s health and plans to broaden her education in this area of P.T.

Monica has 2 small children and lives in Clarksburg with her husband.

Tracey Cannon, MPT

Tracey Cannon received her Masters in Physical Therapy from the University of Maryland at Baltimore in 1997. She was the 1997 recipient of the Pekar-Abell Pediatric Scholarship for outstanding performance in study of pediatric physical therapy.

During her undergraduate education at the University of Maryland Baltimore County she was a Presidential Scholar and an Honors College Scholar. Tracey began her career as a physical therapist working for Easter Seals, more recently she was employed by the Frederick County Infants and Toddlers Early Intervention Program. She meets the Maryland State Department of Education's suitable qualifications requirement for early intervention personnel. Her background is not limited to pediatrics and she has experience with a variety of patients, including the geriatric and memory impaired populations.

Holley Hovermale, DPT

Holley Hovermale received her Bachelor of Science in Health Education from the University of Georgia in 1998 where she received recognition as a Presidential Scholar and was an active member of a national honor society.

She received her Master of Physical Therapy degree from Emory University School of Medicine in 2002, and went on to complete the requirements for the Doctorate of Physical Therapy degree received from Emory University in 2004.

She has practiced primarily in the acute care setting where she has had a wide variety of experience handling patients in a level 2 NICU, adult ICU, a transitional care unit, hospital-based outpatients, and on general med-surg, oncology, or cardiac units. She is skilled in wound care, has a passion for pediatrics and an extensive background working with children as a gymnastics coach for nine years. Holley lives in Rocky Ridge, Maryland with her husband and two sons. In her spare time, she and her husband both run home-based businesses, and enjoy spending time with their children.

Ellen Pape, MAPT, CDT/MLD

Ellen Graduated from Touro College in 1993 with a Masters in Physical Therapy. She earned a Bachelor of Science Degree in Kinesiology from the University of Colorado in 1989.

Ellen has worked in a variety of patient settings, to include several start-up clinics for which she was largely responsible. She enjoys patients of all ages, but has a particular fondness for the geriatric population. Additionally, she has been focused on Women’s Health to include incontinence, fibromyalgia, and Lymphedema. Ellen is certified in Complete Decongestive Therapy and Manual Lymph Drainage.

Ellen has been providing therapy in a home health setting for 4 years and previously worked as the Administrator for a home health organization before moving to Maryland. Currently, she provides services primarily in Prince Georges County.

She lives in Bowie with her husband and daughter. In addition to her contract therapy company, Ellen also owns both a Mary Kay and Gift Basket Business.

Jack Zygmunt, MPT

Jack graduated from Lock Haven University with a Bachelor of Science degree in Health Science. In addition to competing in collegiate athletics as a member of the football team, his studies were focused in the area of sports medicine. In 1994, Jack received his Master of Physical Therapy degree from the University of Pittsburgh.

He has worked in a variety of settings including outpatient orthopaedics, acute care in the Veteran’s Administration hospital system and home health. Jack’s physical therapy interests are in orthopaedics and it’s application to the geriatric population. More recently, he has spent three years consulting with orthopaedic surgeons fellowship trained in the area of joint replacement surgery. During this time he was involved in the preoperative planning for hip, knee and shoulder replacement surgeries as well as being a resource for the operating room staff during such surgeries.

This experience has enabled him to have a unique perspective on the aftercare involved for the patient who has undergone total joint replacement.

Jack is pursuing his Doctorate of Physical Therapy degree to expand his fund of knowledge in his areas of interest, orthopaedics and geriatrics. He lives in Winchester, VA with his wife and two children. As a family they enjoy camping and skiing. Jack is an avid golfer.

Tom Westwood, PTA

Tom Westwood graduated from Frostburg State University in 1991 with a Bachelor of Science degree in Health and Physical Education with a concentration in Health Fitness. Tom worked as a fitness instructor and personal trainer for four years before going back to school to be licensed as a Physical Therapist Assistant.

Tom has spent the last five years in Assisted Living facilities and Nursing Homes working as a Physical Therapist Assistant. In 2001, Tom started his own company, Westwood's Geriatric Care Services, LLC, which provides transportation, companionship and Therapy services to the seniors in Frederick County.

Tom lives in Myersville with his wife and two kids and he enjoys going on bike rides with his family along the C&O canal.

Dr. Carol Hamilton Zehnacker, PT, DPT, CMT

Dr. Carol Hamilton Zehnacker is the owner of Physical Therapy Consults. She is both a licensed physical therapist and a certified personal trainer. She was the first physical therapist in Frederick to become a certified specialist in orthopedic physical therapy (APTA) and manual therapy (Paris approach).

Dr. Zehnacker received her doctorate of physical therapy from Widener University and a master's degree in business administration from Hood College. She has worked in a variety of settings including the Director of Rehabilitation at Frederick Memorial Hospital, skilled nursing facilities, home health for Frederick and Washington Counties, and her own private practice. She assisted in the development of the Wellness Program at Washington County Hospital and has designed a Back School Program presented to over a thousand patients including members of seniors center and assisted care living facilities. At present she is also teaching at Frederick Community College Continuing Education in the Institute of Learning in Retirement where she lectures on musculoskeletal dysfunction or simply coping with the aches and pains of everyday life in the aging process.

Her special interests are in the care and treatment of osteoporosis and the incorporation of an active and healthy lifestyle in the older population. She is an avid golfer and has developed "Golfit" a program of golf injury prevention that relieves the stresses of the golf swing and improved your games. In addition she is on the clinical advisory panel for the American Running Association and serves as chairperson for the Western District of the Arthritis Foundation.

Dr. Zehnacker was the 2005 recipient of the prestigious Kendell award presented for outstanding service to the profession and to the public.

Kristy Bugbee, MPT

Kristy graduated from St. Ambrose University, in Davenport, IA in 1996 with a Masters Degree in Physical Therapy. Prior to that, she received her Bachelors Degree in Biology from the University of Iowa as well as completing multiple graduate courses in Counseling. She began her physical therapy career primarily in inpatient rehabilitation where she developed her skills with multi-trauma patients including traumatic brain injuries, spinal cord injuries and multiple fractures. She also treated people following brain tumor resections, lower extremity amputations, joint replacements and Phase I cardiac rehab. She gained experience with aquatic therapy and wheelchair seating and positioning. Her primary area of interest has been Neurological Rehab. She further advanced her knowledge through working in long term care, sub-acute settings and outpatient services. She has seen the entire continuum of care from hospital to home. She believes HomeCare Rehab and Nursing, LLC is the ideal setting for her to be able to utilize all of her skills in the most functional setting, one’s home. Kristy is also eager to expand this knowledge as the field of rehabilitation is forever advancing. In her spare time she enjoys spending time with her new puppy, Hawkeye.

JayneJayne Gribble, PT

Jayne joins HomeCare Rehab and Nursing with 23 years of diverse outpatient clinical experience. Jayne graduated from St. Louis University with Honors and has extensive experience with joint replacements, rotator cuff syndromes/surgery, postural dysfunctions, spine care and education as well as a variety of neurological conditions. She resides in Germantown Maryland with her daughter Dylan and stays active so she can have health with longevity, the better to be of service to her patients and clients.

TammyTammy Spitzer, PT, GCFP

Tammy has nearly 25 years experience as a Physical Therapist serving this community. She is also a Guild-Certified Feldenkrais PractitionerTM and a certified Sounder Sleep InstructorTM. She enjoys helping others take control of their own healing potentials to eliminate stress and pain, and maximize their fullest potential. She has provided therapy in many settings including; out-patient, home-health, nursing homes and rehab centers. She is a certified Brain Injury Specialist. Tammy is also certified in many specialty areas; such as, Neuro-Developmental Treatments (NDT), Proprioceptive Neuromuscular Facilitation (PNF) techniques and Myofascial Release (MFR), among others.

Tammy has also taught neurology and wound care with Shenandoah University's Physical Therapy School and wound care at University of Maryland's Physical Therapy School. She has worked with clients of all ages, with various health concerns such as; managing severe chronic illness and disability, reducing pain and discomfort, improving balance and daily functioning and promoting healthy aging. Tammy also teaches regular weekly group Awareness Through MovementTM classes and special workshops for diverse populations through the Feldenkrais MethodRT. She has taught Awareness Through MovementTM classes through the Commission on Aging, Sports Connection Health Club, Rehoboth United Methodist Church and through Hagerstown Community College.

Tammy also has 2 awesome children, named Matthew and Zachary, and a wonderful husband, Chuck. She has always lived in the Washington County area, and enjoys the outdoors, reading and especially family time.

Danielle Kinnamont, PT

Danielle Kinnamont received her Bachelor of Science in Health Science from Saint Francis University in Loretto, PA in 2001. During this time period, Danielle participated in numerous volunteer activities, including being president of her campus chapter of Habitat for Humanity. She then received her Masters in Physical Therapy from Saint Francis in 2003.

She has worked in the acute care hospital setting since graduating. However, due to scheduled six month rotations, she was able to gain experience with acute, subacute, and also outpatient therapy. She assisted in developing a new therapy program for the special care nursery within the hospital and was also the lead therapist during the implementation of a new, more aggressive total joint replacement program. Danielle especially enjoys working with patients with amputations, the geriatric population, and those who have recently undergone a joint replacement. She resides, in New Market with her husband and two young children. She enjoys spending time with friends and family both locally and out of state, as well as participating in her parish bible study.

Jin Park, MSPT (Photo coming soon!)

Jin graduated from Ithaca College with a Bachelor of Science and Masters of Science in Physical Therapy in 2002. As a student, he was involved in clinical research involving ACL injuries and knee and foot biomechanics. His thesis focused on affects of the knee with varying heel and toe landing with stepping down. He also competed as a diver at the collegiate level.

Jin began his career in NC working in outpatient orthopedics, as well as acute hospital care, which exposed him to the spectrum of rehabilitation. Thereafter, he moved to Wyoming, working in a manual-based outpatient orthopedic setting, while also establishing their aquatic therapy program. He saw a variety of patients including post-surgical, spinal, peripheral joint, and vestibular/balance dysfunction, as well as stroke, and Parkinson’s patients. He is now excited to provide home health services in Maryland, which allows the one on one treatments that outpatient often doesn’t provide.

Jin has interests in pursuing his Doctorate of Physical Therapy and becoming a Certified Specialist in Geriatrics or Orthopedics. He lives in Frederick, MD and enjoys fly fishing, athletics, hiking, and travel.

Bob Kropkowski, PTA

Bob Kropkowski, PTA, CMT, Bob received his B.A. in Sociology from the University of Baltimore in 1973. He earned his Physical Therapist Assistant degree at Carroll County Community College in 1998 where he was president of Phi Theta Kappa Honor Society and an All-USA Academic team award recipient. He also attended the Baltimore School of Massage and received his Certified Massage Therapist designation in 2001.

Bob's PTA experience has been in outpatient, assisted living and nursing home facilities, but prefers the patient benefits that only home care can offer. He has been working in home health for the past 7 years.

Bob's massage company, Professional Therapeutic Massage, provides home massages and corporate seated massages. You may see him perform on piano and vocals at several assisted living facilities in the area. He lives in Westminster with his wife and two children. He enjoys running on some of Westminster’s rural farm roads in the early morning.

Cynthia Breyer, DPTDr. Cynthia Breyer, DPT

Cynthia Breyer, DPT, B.S. received her Bachelor of Science degree from Duke University in 1998 followed by a Doctorate in Physical Therapy from Arcadia University in 2003, graduating with honors. She has worked primarily with the geriatric population in a community-based hospital setting for the last four years treating a wide variety of acute and chronic conditions. Most recently she was appointed Clinical Specialist at Suburban Hospital in Bethesda, MD serving as the coordinator for the hospital’s Balance/Fall Prevention Program. She gave monthly lectures and balance screens while leading functional exercise classes at local senior centers. In 2003, Cyndi had the privilege of providing pro-bono physical therapy services for impoverished citizens in St. Elizabeth Parish, Jamaica as part of her clinical training, which served as an invaluable exposure to the challenges of home therapy in a unique setting.

Cyndi has a keen interest and love for the geriatric population which, in addition to her physical therapy career, lead her to start the Age to Age elderly ministry for her local church organizing weekly Bible studies and an Adopt-a-grandparent program. She has also served as a Hospice volunteer for Frederick & Montgomery counties for the last three years. Cyndi has a passion for Christian international missions and most recently has traveled to Nepal and Rwanda to engage in church planting, evangelistic & AIDS awareness campaigns. In her spare time, she loves reading, playing piano, tennis, golf & field hockey, as well as hiking & mountain biking. Cyndi is a former Division 1 Field Hockey All-American at Duke and would love to coach someday at the high school level. Cyndi currently resides in Bethesda, MD.

Teresa Bowie, PT

Teresa graduated from the University of Pittsburgh in 1988 with a Bachelor's degree in Biology. After working in cancer research for several years, she attended Physical Therapy school at the University of Maryland Eastern Shore and received her Master's in Physical Therapy in 1997.

  Teresa has over 9 years of experience working in an outpatient orthopedic setting. She has worked with a wide variety of patients including total joint replacements, sports injuries and worker's compensation. She also has training in Women's Health, Vestibular Disorders, Water Therapy, Parkinson's Disease, Shoulder Rehabilitation and Knee Rehabilitation.

Teresa lives in Frederick with her husband and two children.

Eric Anson, PT

Eric Anson, PT, received his Baccalaureate of Science in Kinesiology from the University of Houston in 1999 and his Masters degree in Physical Therapy from Texas Tech University Health Science Center in 2003. His master’s research project resulted in a published article on improving the reliability of manual mobilization of the lumbar spine titled The Use of an Educational Model in the Improvement of Student Reliability in Finding R1 in The Journal of Manual and Manipulative Therapy in 2004.

Eric specializes in the treatment of balance impairments with a special interest in vestibular disorders. He successfully completed the Susan Herdman Competency in Vestibular Rehabilitation at Emory University in 2006. He co-developed the Better Balance Program at Lake Charles Memorial Hospital in Lake Charles, LA in 2005. Eric has designed and performed clinical research involving fall risk assessment for individuals with balance impairments.

In addition to being a clinical instructor to physical therapist/ physical therapist assistant students, Eric taught as a guest lecturer the vestibular rehabilitation component of the neurological rehabilitation class at Marymount University in 2007.

Eric is currently enrolled as a doctoral student in the Kinesiology department at the University of Maryland where he continues to research postural control and vestibular disorders.

Eric has a wife of 4 years. He enjoys hiking and Detroit RedWings Hockey.

Speech Therapists

Karen J. Sowers, Speech Pathologist and founder of Better Speech and Swallow Corporation

Karen J. Sowers, M.S. CCC-SLP received her Masters of Science in Speech Language Pathology from Towson University in 1993. Initially starting her career in Skilled Nursing and Sub-acute care, she established a firm foundation and passion for the treatment of swallowing disorders. Karen subcontracted for Maryland Neuro-Rehab Foundation working with head injury and strokes clients prior to coming to work in Acute Care at the beginning of 2000. Gaining acute care experience, including Modified Barium Swallow proficiency in Radiology and multi-complex care in ICU, she took this knowledge and began mentoring and instructing less experienced clinicians and student speech therapists. Most recently, prior to founding her own company, Karen managed the Rehabilitation Department for Frederick Memorial Hospital including inpatient, the sub-acute unit and campus outpatient program. During this time she has gained certification in both DPNS (Deep Pharyngeal Neuromuscular Stimulation) and Vital Stim, both used in the treatment of swallowing disorders. Karen not only has practiced in the Frederick area for over 10 years, she is also a local resident. When not at work she enjoys spending time with her husband and 4 children.

We are pleased to have Karen from Better Speech and Swallow Corp., teaming with HomeCare Rehab and Nursing to provide speech services.

Occupational Therapists

Sue McShane Paul, Senior Occupational Therapist , OTR/L

Sue McShane Paul is the Occupational Therapist and the Chief Operating Officer at HomeCare Rehab and Nursing in central Maryland. Ms. Paul has extensive experience as an occupational therapist working with adult patients with all types of physical and neurological diagnoses. She was Board Certified in Geriatrics through the American Occupational Therapy Association from 2001 to 2007 and has specialized in the management and treatment of persons with dementia for over 10 years. Sue has been very active with the American Occupational Therapy Association as a member of the nominations committee for the geriatric special interest section. She served on the National Alzheimer’s Association Committee for end of life practices in 2006. Sue is the creator and author of the Head, Hands, and Heart Program which is the cornerstone of dementia therapy with HomeCare Rehab and Nursing. Sue Paul can be reached at our office: 301-662-1997.

Mary Schwartz, MS OTR/L

Mary is an occupational therapist with over 7 years experience in a combination of home health care and outpatient settings. She received her undergraduate degree from the University of Delaware and completed her Masters Degree in Occupational Therapy at Towson University. Mary has worked with all populations and varied diagnoses throughout her career although she has a special interest in neuro rehab and low vision. While working in an outpatient setting in Hagerstown, Mary was instrumental in the development of driving rehab and low vision programs.

She is currently working on launching a new low vision program through HomeCare Rehab and Nursing, LLC which will provide much needed low vision services to clients in the community with severe visual impairments. Mary lives in New Market with her two small children and husband.

Kathy Luehman, OTR/L

Kathy brings 22 years of experience as an Occupational Therapist. She graduated with a Bachelor of Science in Occupational Therapy from Medical College of Virginia in 1983. She has spent the past 15 years serving home health care clients in Frederick county and has treated a diverse patient population with neurological, orthopedic, oncology and low vision diagnoses. Prior to working in home healthcare, Kathy worked in psychiatric, pediatric, rehab, long term and acute care settings. She has focused her practice in home healthcare because that is where she believes her skills as a therapist are effective. Kathy lives with her husband and two teenage children and enjoys reading, swimming and hiking.

Theresa Davis, OTR/L

Theresa graduated from Towson State University in 1994 with a Bachelor of Science in Occupational Therapy. She began her career in Acute Rehab at National Rehabilitation Hospital in D.C., working with both neurological and orthopedic patients. She then moved to the acute care/sub-acute setting at Frederick Memorial Hospital, treating all diagnoses of patients including; stroke, cardiac, Parkinson's, Multiple Sclerosis, joint replacement and amputees. Theresa has a particular interest in the geriatric population. She then moved to the homehealth care setting where she has been treating patients in their homes since 1998. She believes her skills are most effective in this setting. Patients are seen individually with specific treatment plans designed and implemented to help them reach their optimal functional independence. She specializes in neurological rehabilitation and home safety assessment and modification. Theresa is also involved with the Department of Aging in providing environmental assessments to keep clients in their homes. She resides in Mount Airy with her husband and two young boys. She enjoys water sports/boating and spending time with her family.

Barri Hoffrichter, OTR/L

Barri Hoffrichter has been working as an Occupational Therapist since 1993. A native of Richmond, Virginia, Barri received her Bachelor’s degree in Human Services from Elon College in 1990, after which she continued on to pursue training in Occupational Therapy. She received her Masters of Science in Occupational Therapy in 1993 after three years of post-graduate study at Florida International University. She has an extensive background in the treatment of both adults and children, and has been treating “home health” patients since 1998 with problems ranging from uncomplicated to severe.

Barri lives in Frederick County and is a proud wife and mother of two wonderful children.
